Photocopy of Diploma Mark sheet & Certificate

Certificate of work experience certificate of working in India

Three colored passport size photograph

Rs. 30.00 stamp with Self addressed 10x12 inches envelope

File Cover with tag

Registration Fees:

Paramedical Staff for Rs. 3000

Application for registration can be obtained from the office of Indian ParaMedical Faculty located at: K-104 Raj Retret Appartment

Rs. 500.00 Bank draft, in favour of Secretary, Indian PARA Medical Faculty.

Self attested four colored passport size photograph

Four specimen signature at plain paper

Domicile certificate

High school, Intermediate mark sheet and self attested photocopy of pass out Paramedical diploma

Self attested photocopy of diploma certificate

Pledge certificate on stamp paper of Rs. 10.00

Certificate related with Planning/employment

Two envelop 10x12 one plain & one with RS. 30 Stamp
